Dear Director:

I wish to extend my thanks and appreciation to the staff who worked with my cousin, E.G.

After his hospitalization, E. was in your facility from January 6 to February 9, 2018. Thanks to the care he received, he was able to return home and he continues his life as previously. He is now almost 95.

As I have visited E. frequently and at different times of the day, I was able observe the staff as they worked, and they should be commended!

I would appreciate you letting the staff involved in E.’s care know they were mentioned in this letter and that their helpfulness was noted and appreciated.

Obviously, you as the Director, have set a commendable atmosphere and high standards for your staff to follow.

With appreciation, G.B.

Hi Carol,

Hope you had a great holiday. It was SO WONDERFUL to have my Dad home with me and my family – thanks for your suggested help with the van service. He was my Christmas miracle!

Your PT team in Dileep and OT Brandon are amazing and I can’t believe in just 4 weeks he has made this advancement. We have not had the right care for him UNTIL his PT team at Fulton and so I need to shout this great news from the rooftops!! Please thank the people who hired and placed your amazing PT/OT team as they are literally doing God’s work.

The HOPE your team is giving my father is worth more than gold. He even told his friends and family by phone “If I was here sooner, Id be walking”. He added he “is seeing people in their 80’s getting PT and getting well in days going to the training room”. These are words Dad never had the confidence to utter until now and I am SO GRATEFUL!!

He is excelling from where he was 4 weeks ago and Dr. Kamal’s suggestion to change his medication has helped in PT participation greatly!

Monica, your nutritionist spotted he was on only 1000 calories a day prior to coming to Fulton and upped it to 1800 on a new formula. I have seen his famished legs regain muscle and tone as he had lost 20 pounds from his 190 lb. healthy weight and it is getting back up there now.

I am looking forward to more progress for him ahead and for a much-desired spring time weather!

To see NOTHING at all on that left side and see it come alive is beyond WONDERFUL!!

Happy New Year…and for Dad it truly is!! M.

Patti,

Thank you so much for all your hard work and perseverance in helping me get my mom back home…you have put a smile on her face again!

Everyday people are writing letters to complain about bad customer service, poor product quality, etc. It’s not that often one has the opportunity to write a thank you note to some one that went the extra mile and provided such great customer service – so I wanted to take the time to do just that.

You have been extremely helpful and truly understood our family’s goals concerning my mom’s care. For once I felt like I was not alone as her advocate!

You do a fantastic job working with families to get their loved ones home, ensuring every detail is buttoned up for a smooth transition back into their community.

Thanks again for all your help – we greatly appreciate it! D.S

Dear Ms. Doyle

On behalf of the entire G. family, I would like to extend my deepest gratitude to your facility, specifically the staff on 2 West and 4 West.

The care my wife, L.G. has received as a resident has been commendable.

Her needs, both physical and emotional have been met with consistent excellence.

Your facility has acted as an extension of our family and cared for Mrs. G. with the highest regard and utmost compassion. We appreciate more than can be expressed, your remarkable commitment to preserving the integrity of these in your care, while seeing to daily and medical needs as well.

Thank you so very much.

With deepest regards, A.G
To all the Staff

I want to thank everyone at Fulton Commons Care Center for taking care of my wife for the past 6 years.

She passed away on April 10, 2011, and I want to express my gratitude for the care and attention that my wife received under your care. It was always comforting to know that whatever questions I had were always answered, and the support that you gave me is profoundly appreciated.

I will always remember all of you for taking care of my wife. May God bless all of you and your families with health and happiness

Sincerely, G.G

Dear Mr. Norton

I was discharged on January 29, 2011 after 4 + weeks in your Rehab Unit.

Originally it was expected that it would take me at least 6 weeks to be able to improve enough to go home; but the extraordinary physical therapist I was assigned to JV was so warm, responsive and inspiring I was able to exceed all expectations. I hope you will share this with her; she is an absolute gem!

All the Occupational Therapists were focused on keeping me cheerful and strong, and they succeeded admirably. KP and CJ, the two transportation aides were cheerful, respectful and gentle; I looked forward to them coming to pick me up and take me to therapy.

MW, the charge nurse was extremely helpful and responsive to any needs I expressed and VP was unflappable when everyone wanted their meds at the same time.

The CNA staff was as responsive as possible; sometimes it was difficult since many of the patients had extreme needs that all had to be met at the same time.

I am delighted to be home, but happy I chose your facility.

Sincerely yours, MGF
Dear Mr. Norton

There are no other words that I can think of except Thank you one and all. You and your entire staff are the most wonderful dedicated people I have ever met. Not one of you would hesitate to go out of your way for others. Not only was my mother BR treated with dignity and respect she was loved by all. During her last days at Fulton Commons it was amazing, while I was sitting with Mom, how many staff members came in to see how she was doing and would say a little prayer for her. These were people who I never met and after her being here for six and a half years I thought I knew everyone. From the man in the kitchen to the housekeeping staff to the beauty parlor staff and to all the resident’s family members, it was unbelievable how much she was loved.

And I have the utmost respect for each and every one of the staff members and also you for your good hiring skills. May you all stay well and God Bless and Thank you for everything and for being you.

Love each and every one of you with all my heart, VL
Dear Greg Norton

I would like to extend my deepest appreciation and gratitude to all your staff at Fulton Commons Care Center in East Meadow, especially DW, LPN and AF, Director of Social Services. I can not tell you what a stressful and emotional time this has been for our family and having D and A’s constant professional care as well as gentle manner towards my family and myself has been a tremendous relief. They have taken such good care of my mother, KF. As a result she is finally coming home. I would greatly appreciate it if you could extend my sentiments to both D and A, as we are so grateful.

Kindest regards, RZ
Dear Ms. Tansi

I would like to begin by apologizing to the staff of 2 West for the tardiness of this letter. However, Fulton Commons has consistently been in our thoughts.

Our family wishes to extend our sincere thanks for the wonderful care provided to our father, JW during his three-year stay at Fulton Commons. More importantly the care during the final week of our father’s life was an example of how palliative care should be provided by nursing home personnel. The entire three shifts provided compassionate care not only to my father but to his family as well. We were provided with 24-hour access to support our father during his final days. Every staff member was professional and emphatic to our feelings and concerns.

We extend our extreme appreciation to everyone who was part of our father’s treatment. We wish the entire staff a happy and healthy holiday season.

Sincerely, W Family
Dear Mr. Norton,

My mother is M, and I would like to take this opportunity to thank both you and your staff for providing the Fulton Commons residents and their families with the “Family Picnic Day”. It certainly was apparent that all of you worked so very hard to ensure the success of the event. It was beautiful, the music was great, the food was delicious, and the staff gave it their all. I especially want to thank all of you, and acknowledge the fact that you gave up your weekend family time to be with our family – it meant so much.

Once again, thank you for being my mother’s “extended family” during such a precious time in her life. Her caregivers at Fulton Commons are truly wonderful and very dedicated in providing the best care possible.

Sincerely, GP
